Abstract: The National Policy for the Comprehensive Health of the Black Population (PNSIPN), established in 2009, represents a milestone in recognizing racism as a social determinant of health in Brazil. However, its implementation remains limited and uneven across the country. This study conducted a qualitative documentary analysis of technical reports, public databases, and institutional documents produced between 2018 and 2025 to identify advances, gaps, and challenges in the policy’s execution. The findings reveal low institutionalization of the PNSIPN, reflected in the small number of municipalities that have implemented it, the absence of specific coordinating structures, weak monitoring mechanisms, and insufficient race disaggregated data. The high rate of non response to the national survey and the indifference expressed by municipal managers highlight institutional disengagement and the persistence of institutional racism as a structuring force in public health governance. The study concludes that the PNSIPN remains vulnerable to administrative discontinuity, lacks stable funding, and depends on isolated initiatives. Strengthening governance, ensuring continuous financing, expanding transparency, and explicitly confronting institutional racism are essential to advancing racial equity within Brazil’s Unified Health System (SUS).
