IMPLANTAÇÃO DOS CUIDADOS FARMACÊUTICOS NO PROCESSO DE ALTA HOSPITALAR NO PACIENTE TRANSPLANTADO: RELATO DE EXPERIÊNCIA
A alta hospitalar inserido no contexto do paciente transplantado, é um ato de responsabilidade multiprofissional, onde o farmacêutico impede que erros e danos alcancem os pacientes. O objetivo do estudo é descrever a experiência da implantação dos cuidados farmacêuticos no processo de alta hospitalar no paciente transplantado renal. Trata-se de um estudo descritivo, tipo relato de experiência, desenvolvido a partir da vivência e experiência na Residência Multiprofissional em Transplante de Órgãos e tecidos, em um Hospital de referência no estado do Ceará. A primeira alta pós transplante é bastante completa e complexa para o paciente, perpassando por todas as categorias. O farmacêutico possui habilidade e competência para realizar análise da prescrição médica, mediante esta análise o profissional realiza a conciliação medicamentosa de alta. O paciente em sua primeira alta necessita receber todos imunossupressores para dá continuidade à terapia de manutenção. Deste modo, pode-se concluir a importância do residente farmacêutico no âmbito assistencial e sua competência e habilidade em contribuir no plano terapêutico de alta, com orientações adequadas e seguras, além de desenvolver metodologias para adesão a terapêutica.

Abstract:
Hospital discharge inserted in the context of the transplanted patient, is an act of multiprofessional responsibility, where the pharmacist prevents errors and damages from reaching the patient. The objective of the study is to describe the experience of implementing pharmaceutical assistance in the discharge process in kidney transplant patients. This is a descriptive study, type of experience report, developed from the multiprofessional residency in Organ and Tissue Transplantation, in a reference hospital in the state of Ceará. The first discharge after the transplant is quite complete and complex for the patient, covering all categories. The pharmacist has the capacity and competence to carry out analysis of the medical prescription, through this analysis the professional performs the reconciliation of the discharge of the medication. The patient on his first discharge must receive all immunosuppressants to continue maintenance therapy. Thus, it is possible to conclude the importance of the resident pharmacist in the assistance scope and of his competence and ability to contribute to the discharge therapeutic plan, with adequate and safe guidelines, in addition to developing methodologies for adherence to therapy.
