Abstract: Depressive disorders are among the leading factors associated with poor therapeutic adherence in chronic diseases, including ocular conditions. Disorders such as glaucoma, diabetic retinopathy, and macular degeneration require long-term follow-up and strict compliance with medication, appointments, and monitoring. However, patients with depressive symptoms show reduced motivation, concentration, and self-care awareness, impairing disease control and increasing the risk of irreversible complications such as blindness. This narrative review aims to analyze the relationship between depression and treatment adherence in chronic ocular diseases, discussing neurobiological and psychosocial mechanisms involved and emphasizing the need for integrated care between ophthalmology, clinical medicine, and mental health to improve therapeutic outcomes.
