HEMATOMA PAROXÍSTICO DIGITAL (SÍNDROME DE ACHENBACH) PAROXYSMAL FINGER HAEMATOMA (ACHENBACH SYNDROME)
A Síndrome de Achenbach, uma condição benigna e autolimitada, é marcada por episódios agudos de mudança de cor, edema e dor dos dedos. Diante disso, o clínico não deve se esquecer de incluir essa condição médica em seu leque de diagnósticos diferenciais. O relato descreve uma mulher de 51 anos apresentando dor em alta intensidade em sua mão, associado a crises de edema e mudança de cor em seus dedos. Durante a investigação, não houve alteração dos exames laboratoriais para Hepatite B e C e HIV. Também vieram negativas as condições de doenças associadas ao Lúpus e às vasculites, fechando o diagnóstico para Síndrome de Achenbach.

Abstract:
Achenbach Syndrome, almost known as paroxysm finger haematoma or acute blue finger, is a benign condition that occurs with a color change in the finger, especially the hands associated with pain and edema. The local normally affected is the intermediate portion of the phalanges, and in general, occurring in only 1 finger at a time; all of fingers can be involved. In all cases, the resolution is spontaneous and self-limited, lasting an average of 3 to 6 days. It’s a rare condition, with approximately 100 cases described in the literature, which mainly affects middle-aged women. The pathophysiology is unknown, as well as its causes, but there is the hypothesis that exists a local capillary fragility. The diagnosis is eminently clinical, based on the signs and symptoms present: cyanosis, edema, pain, paresthesia and pruritus. Complementary exams are normal and the treatment for this condition is only symptomatic for pain.
