Abstract: Understanding the aging process means understanding the person heuristically. Realizing that the institutionalization process confronts the elderly person with an unknown reality that they may not be able to deal with, the intervention of the Specialist Nurse in Mental and Psychiatric Health Nursing (EEESMP) becomes necessary, providing person-centered care in your circumstances. The aim of this project is to continuously improve the quality of nursing care aimed to understand how the elderly person's transition between home and nursing home is configured, with attention to the nursing focus “anxiety” and “hope”. In this case, two elderly women were accompanied in 3 individual sessions (with the presence of members/family members, if available), in which the following topics were addressed: 1) Remembering memories of the past; 2) Living the present better; and, 3) Plan the future. As a result of the intervention, there was an improvement in anxiety and hope scores during the transition process. Participants and their families expressed their pleasure at having been part of the project, recognizing its importance in promoting hope and meaning in life. The EEESMP intervention proves to be differentiating for elderly people in the process of institutionalization, culminating in positive adaptation processes and gains in mental health.
