Abstract: Obesity is a chronic disease derived from the dysfunction of energy homeostasis, in which there is an imbalance between what is ingested food and energy expenditure. With the alarming increase in diagnoses of obesity cases, the number of comorbidities presented by these patients, such as the presence of metabolic syndrome (MetS), is increasing. With the increase in studies in the area of obesity, the role of genetic bases in susceptibility has been understood, making it possible to identify polymorphisms in genes related to different traits of adiposity and other physiological conditions. Furthermore, the individual's lifestyle, microbiota, socioeconomic status, and cultural factors are also associated with the development of this pathology and its comorbidities. This chapter seeks to expose the epidemiology of obesity and MS, its different aspects added to the stratification of patients, the intrinsic and extrinsic factors to its development and the possible medical approaches to be adopted for the most appropriate and personalized treatment for patients, and future perspectives.
