FACILIDADES/DIFICULDADES AO INICIAR ACOMPANHAMENTO DE SAÚDE EM SERVIÇO ESPECIALIZADO: O OLHAR DO PACIENTE ESTOMIZADO
RESUMO: Objetivo: Explorar quais são os dispositivos facilitadores e dificultadores de acesso ao cuidado especializado do paciente estomizado na rede de serviços especializados em cuidados em estomias no município de Porto Alegre, Rio Grande do Sul. Revisão bibliográfica: A estomização pode levar a complicações que vão muito além das necessidades fisiológicas do cuidar, podendo abranger outros aspectos fundamentais à condição humana. Devemos considerar o paciente de forma holística e humanizada, abrangendo também sua família, com todas as suas expectativas, angústias e necessidades. O paciente estomizado necessitará de novos conhecimentos em relação ao seu corpo e cuidado pessoal e os serviços de saúde, especialmente os ambulatórios especializados, necessitam estar preparados para absorver esses pacientes após a alta hospitalar, acompanhar o seu desenvolvimento e adaptação ao estoma e aos dispositivos desta, bem como prepará-lo para gerir o seu cuidado e/ou o cuidado através do cuidador. Metodologia: Estudo de natureza exploratória e descritiva, com abordagem qualitativa, realizado com 13 pacientes recém estomizados, maiores de 18 anos de idade, no Serviço de Estomatoterapia Vila do Comerciários, vinculado à Secretaria de Saúde do Município de Porto Alegre. A coleta de dados se deu através de entrevista semiestruturada, realizada em três meses no ano de 2019, tendo como recorte de amostra a temporalidade da coleta e ou alcance do número da amostra pretendida. Considerações finais: É fundamental que mais políticas públicas sejam criadas para possibilitar aos pacientes com estomias um acesso mais homogêneo aos serviços existentes, promovendo a redução dos estresses gerados após a alta hospitalar, estimulando melhor integração da rede de cuidado especializado com a atenção primária e hospitalar, com objetivo de diminuir o trânsito dos pacientes pelo território na busca do seu cuidado.

Abstract:
ABSTRACT: Objective: to explore what are the means that facilitate and hindrances to specialized care for the stoma patient in the network of services specialized in stoma care in the city of Porto Alegre, Rio Grande do Sul. Bibliographic review: Ostomy can lead to complications that go far beyond the physiological needs of care, and may encompass other fundamental aspects of the human condition. We must consider the patient in a holistic and humanized way, including, in addition to him, his family, with all their expectations, anxieties and needs. The ostomy patient will need new knowledge regarding their body and personal care, and health services, especially specialized outpatient clinics, need to be prepared to absorb these patients after hospital discharge, monitor their development and adaptation to the stoma and its devices, as well as preparing them to manage their care and/or care through the caregiver. Methodology: An exploratory and descriptive study, with a qualitative approach, carried out with 13 newly ostomized patients, over 18 years of age, of a Vila do Comerciários Stomatherapy Service, linked to the Health Department of the Municipality of Porto Alegre, State of Rio Grande do Sul. Data collection took place through a semi-structured interview. The collection was carried out in 3 months, in the year 2019 and thus having, as a sample cutout, the temporality of the collection and/or range of the intended sample number. Final considerations: It is essential that more public policies should be created to enable patients with ostomies to have more homogeneous access to existing services, promoting the reduction of stress generated after hospital discharge, encouraging better integration of the specialized care network with primary and hospital care with the objective of reducing the transit of patients through the territory in search of their care.
