Abstract: Introduction: The relationship between chemical dependence and accountability is a complex issue affecting millions of people worldwide. Chemical dependence is considered a "Behavioral and Mental Disorder," classified by the WHO as ICD F19. Although national legislation acknowledges chemical dependence as an individual's innocence, the criminal system still faces significant penalties for crimes committed under chemical dependence. Therefore, a careful and individualized approach is necessary to address this complex issue. Objective: To understand the relationship between chemical dependence and accountability. Methodology: This is an integrative literature review, from a qualitative perspective, based on data collection from articles on the Virtual Health Library of the Ministry of Health, PubMed, and Google Scholar. Health descriptors such as "Substance-Related Disorders," "Accountability," "illicit drugs," and "Alcohol-Related Disorders" were used. Scientific articles written in Portuguese, English, and Spanish, related to the delimited theme and in the format of systematic review, books and documents, clinical trial, meta-analysis, analysis, systematic review were included. Publications that did not include the population or approach to the research topic were excluded. Conclusion: A complex and multifaceted relationship between chemical dependence and accountability is revealed. However, it should not be assessed simplistically but rather considering factors such as dependence, substance impact, and individual understanding and control. Chemical dependence can affect cognitive and behavioral control, leading to accountability in some cases. It emphasizes the need for a holistic and sensitive approach to this relationship in legal and clinical contexts.
