Abstract: Introduction: The medical curriculum reform in Brazil has followed a dynamic and adaptable course to global social and educational changes. Curricular guidelines allow institutions autonomy to define specific objectives and overcome local challenges. Miller's pyramid, a fundamental structure in the development of medical knowledge, is guided by steps that go from theory to actual practice. The simulation of clinical cases in pediatric emergency scenarios is an essential educational tool, promoting critical reflection and integration of multiple scenarios for quality medical training. The use of simulators and mannequins allows the creation of realistic experiences, adjusted to the complexity of the simulated clinical situations. Experience Report: In this context, students in the seventh period of a medical course participated in realistic laboratory simulations, based on the Pediatric Advanced Life Support (PALS) guidelines. Organized into emergency care teams, students faced simulated clinical cases with technological support and direct guidance from tutors. The methodology included prior preparation, active simulation and debriefing sessions. Reflection: Initially challenged by complexity and the need for coordination, students experienced continuous learning and refinement of practical skills. Simulation provides a safe environment for experimentation and in-depth learning, allowing the identification of strengths and areas for improvement in clinical practice. Conclusion: The integration of realistic simulation into the medical curriculum provides robust and adaptive training, preparing students for real situations safely and efficiently. Learning based on lived experiences not only strengthens clinical skills, but also promotes the personal and professional development of future doctors, positively impacting both clinical practice and the well-being of staff and patients.
