Abstract: Exosome-derived products are being used as dermocosmetics, showing promising results in the treatment of skin aging and pigmented lesions. In this context, skin enhancers play a crucial role in improving the extracellular environment, combating irregular pigmentation, inflammation and vasodilation, with beneficial effects on skin aging. This study aimed to synthesize the evidence on the use of exosomes for tissue regeneration and anti-aging, with application in orofacial harmonization (OFA). This is an integrative literature review carried out in January 2025, through a search in the PubMed/MEDLINE, Web of Science and LILACS databases, using the health sciences descriptors (DeCS) “Skin”, “Aging, “Exosomes”, “Stem Cells” and “Extracellular Vesicles”, interconnected by “and/or”. Clinical trials, regardless of language, published in a 10-year time frame (2015 to 2025) were selected. In total, 12 clinical trials were tabulated. In the studies, data collection was carried out in healthy adults, with ages ranging from 28 to 68 years, with the average age being around 40 to 50 years. The follow-up time in the studies ranged from 4 weeks to 12 months, with most focusing on periods of 8 to 12 weeks. The area of greatest prevalence was facial aging, with the combination of treatments such as microneedling, mesenchymal stem cell-derived exosomes and laser therapies being the focus of the interventions. The prevalence of microneedling treatments was evident, appearing in 7 studies, standing out as a promising approach for skin rejuvenation and control of facial aging. Combinations of exosomes with other treatments, such as microneedling and CO2 laser, have been shown to be effective in promoting skin regeneration, increasing elasticity, and reducing scars and wrinkles.
