• Abstract:

  Drug errors involving chemotherapeutics put at risk
  patient safety, which can have serious consequences. the advance of
  knowledge and technologies have provoked numerous changes in the way of
  provide care, making the treatment and rehabilitation process of the
  patient, on an outpatient or inpatient basis, is permeated by a
  series of risks, which can threaten patient safety The study aimed to
  objectives to identify in the literature national and international scientific productions
  on adverse events related to medication errors involving
  chemotherapy in the care of cancer patients; know the factors that
  contribute to the occurrence of these adverse events and identify strategies that
  help to prevent this type of occurrence. The bibliographic survey was
  carried out in PubMed, LILACS, Scielo and BDENF databases. Through
  exhaustive reading and analysis of the scientific production published between the years 2013 and
  2018, the following results emerged: absence of institutional routines,
  unfavorable working conditions, task overload and lack of systems
  that help in the process of prescribing treatment, are the
  main factors that contribute to the occurrence of errors. In contrast, the
  studies show that most events are preventable and point to some
  Strategies for preventing future errors: pharmacist's role in the review
  of chemotherapy prescriptions, implementation of electronic checklists and
  implementation of integrated information systems that support decisions
  physicians and assist in the preparation of the prescription. The importance of
  culture of reporting adverse events, as the analysis of occurrences
  enables the improvement of work processes.
