ETNOGRAFIA DOS SABERES DA FAMÍLIA SOBRE DOENÇA RENAL CRÔNICA, HEMODIÁLISE E CUIDADOS DOMICILIARES
Neste capítulo apresentamos um estudo que objetivou compreender os saberes de familiares de pessoas em hemodiálise sobre o tratamento, a doença renal crônica, e cuidados domiciliares. Realizamos uma etnografia, ancorada na etnoenfermagem orientada por Madeleine Leininger, coletando as informações por observação participante e entrevista em visitas domiciliares a sete pessoas em tratamento por hemodiálise e quinze familiares. Encontramos quatro categorias: “Estou me cuidando melhor, não tô cem por cento, mas eu me esforço”: saberes e práticas de saúde; Doença Renal Crônica:“é assustadora, mas tratando, cuidando é possível viver com ela”; “O socorro que eles dão num primeiro momento é que teria que ser permanente”: experiência da família no serviço de hemodiálise; “Toda vez que você precisa é uma dificuldade danada”: Trajetória da família na rede de atenção à saúde. Os familiares sabem o que é a doença renal crônica, sua predisposição familiar; tratamento e cuidados. A principal fonte são os profissionais de saúde, trocas de experiência com outros pacientes e familiares do serviço de hemodiálise. Compreendemos que mesmo tendo recebido orientações/informações com conteúdo técnico-científico os familiares explicitaram dificuldades em aderirem às práticas de autocuidado. Como considerações finais, apontamos que características culturais prevalecem nos hábitos básicos e modo de cuidar da saúde e, a partir das experiências de busca de cuidados pelos participantes, em outros serviços de saúde, é relevante uma política de suporte à família do paciente Renal Crônico em hemodiálise, sobretudo dos dependentes e que vislumbre a prevenção da continuidade da doença entre os familiares.

Abstract:
In this chapter, we present a study that aimed to understand the knowledge of family members of people on hemodialysis about treatment, chronic kidney disease, and home care. We performed an ethnography, anchored in ethno-nursing guided by Madeleine Leininger, collecting information through participant observation and interviews during home visits to seven people undergoing hemodialysis treatment and fifteen family members. We found four categories: “I'm taking better care of myself, I'm not one hundred percent, but I make an effort”: knowledge and health practices; Chronic Kidney Disease: “It's scary, but treating, taking care of it is possible to live with it”; “The help they give at first is that it would have to be permanent”: family experience in the hemodialysis service; “Every time you need it is a darn difficulty”: The family's trajectory in the health care network. Family members know what chronic kidney disease is, their family predisposition; treatment and care. The main source are health professionals, exchanges of experience with other patients and family members of the hemodialysis service. We understand that even having received guidance/information with technical-scientific content, family members explained difficulties in adhering to self-care practices. As final considerations, we point out that cultural characteristics prevail in the basic habits and way of caring for health and, based on the participants' experiences of seeking care in other health services, a policy to support the family of the Chronic Kidney patient in hemodialysis, especially for dependents and that aims to prevent the continuity of the disease among family members.
