Abstract: Healthcare-associated infections (HAIs) are considered a serious public health problem with significant morbidity and mortality. In this context, the use of antimicrobials plays an important role, although it may be associated with adverse events. Currently, the treatment of infections has undergone changes due to increasing microbial resistance. This represents a global concern, as it is becoming increasingly frequent and serious. The objective of this work was to carry out a descriptive study on the treatment of patients with a positive culture for carbapenem-resistant microorganisms in a university hospital. It was observed that de-escalation, narrowing of the antimicrobial spectrum based on the result of an antibiogram, and the use of combined therapy are practices adopted in these cases. Considering the necessary care to not increase selective pressure on microorganisms and optimize treatments for conditions caused by them, some practices must be implemented, such as de-escalation, recommended by the Antimicrobial Stewardship program. The results of this work may be useful for better understanding care practice and support future improvement strategies in the study hospital.
