Abstract: Warfarin is an oral anticoagulant of great importance in clinical practice for the prevention and treatment of thromboembolic disorders, which has a narrow therapeutic index. Accordingly, small variations in its dose may result in poisoning outcomes or sub-therapeutic dose. Individualized dosing regimens are widely used in the treatment with this anticoagulant. In order to comply with such doses, it is necessary the fractionation of the dosage form for dose adjustment by tablet-splitting. Tablet-splitting is an activity that provides dosage flexibility and cost reduction. However, it can result in incorrect dose administration due to unequal distribution and the presence of drug residues, which can pose a significant risk if the drug is one from a narrow therapeutic index. This is a descriptive study of the administration schemes of warfarin and the frequency of tablet-splitting for patients undergoing treatment in anticoagulation clinic. The divisibility of the tablets determined by prescription was observed in 93% of patients on warfarin use. The inclusion of tablets of 2.5 mg of warfarin in the basic medication catalogue from the Brazilian Unified Health System educes the need for splitting, besides reducing the complexity of its administration process, therefore ensuring greater dose accuracy and indirectly encouraging compliance due to ease of use. There are no studies to date evaluating the clinical impact of the dosing regimen of tablets of split warfarin sodium. Thus, any conclusion about the clinical impact for this agent can be done so far.
