Abstract: Female androgenetic alopecia (FAGA), often associated with telogen effluvium, is a multifactorial condition that impacts both aesthetics and emotional well-being. Factors such as hormonal dysfunctions, nutritional deficiencies, subclinical inflammation, prolonged use of contraceptives, and metabolic alterations contribute to its progression. A rarely addressed challenge is follicular plugging the obstruction of the follicular ostium by sebum and residues, which hinders the absorption of topical treatments and perpetuates inflammation. This clinical case describes a 30-year-old patient with FAGA and chronic telogen effluvium, associated with prediabetes, stress, and contraceptive use. Trichoscopic and macroscopic evaluations revealed miniaturized hairs, perifollicular scaling, and areas with irregular hair density. The integrative therapeutic protocol combined deep cleansing with a desobstructive pre-shampoo, topical caffeine application for DHT blockade, physical stimuli in the clinic, and lifestyle changes. Removal of follicular plugging proved essential to optimize active ingredient penetration, enhancing treatment effects. Adherence to the protocol was strengthened through continuous follow-up and educational guidance. The case demonstrates that success in treating FAGA depends on a personalized approach, correct use of therapeutic resources, and active partnership between patient and professional, offering a safe and effective alternative to oral medications.
