Abstract: This article critically examines the surgical and conservative management of juvenile idiopathic scoliosis (JIS), based on a review of 30 updated and relevant studies. It identifies the criteria guiding surgical decision-making and emphasizes the importance of treatment individualization, brace adherence, and angular curve progression. The research reveals that conservative techniques, such as bracing and scoliosis-specific physiotherapy, achieve high success rates in mild to moderate cases. Surgery, in turn, plays a crucial role in refractory cases, enhanced by modern technologies. The study concludes that the ideal intervention should consider not only radiological parameters, but also functional, genetic, and psychosocial factors. An integrated and personalized vision for JIS treatment is proposed.
