Abstract: Introduction: Person-centred care is related to empowerment in the case of chronic diseases. It focuses on the person/family, the community and decision-making processes related to the promotion of self-care in the management of therapeutic regimes. Person-centred care goes beyond most nursing models, but also beyond health promotion and policy currents. Theoretical Framework: The concepts used in this study were mainly Empowerment. Method: Qualitative cross-sectional study with data analysed according to grounded theory (Charmaz). Nine semi-structured interviews were conducted. Results: The process of building Empowerment was categorised by means of the theoretical explanation: "facilitating the decision according to one's own mind" and was made up of three processes/categories: raising awareness of the need for change; perceiving oneself as being capable of change; deciding to change. This process is made up of phases that need to be resolved in order for the person to move forward and is part of the nurse's intention when developing nursing interventions. Primary healthcare provides an excellent context for the application of person-centred care in chronic illness, due to its proximity to the population (McCormack & McCance (2017). Person-centred care and its needs seem to be part of a theoretical discourse in nursing, but without practical application in the field. Conclusion: The model of person-centred care in chronic illness could give visibility to the nursing discipline, especially in achieving health gains; having implications in terms of personal satisfaction, improved well-being, self-care and prevention of complications: namely in preventing early deaths, reducing the number of hospitalisations.
