Abstract: Objective: To report the case of a young patient who presented immediate pain and erythema in the gluteus shortly after an injection of penicillin and decadron. After medical evaluation, he was diagnosed with Nicolau Syndrome, a rare vascular complication with embolic occlusion of the arteries after the improper administration of an intramuscular injection into the local vasculature. It is a rare syndrome, however its clinical manifestations can lead to necrosis of the skin, subcutaneous tissue and local muscles as an ischemic complication. In some cases, it presents with hypersensitivity, paraplegia and motor deficits due to nerve injury. Method: Descriptive case report with a qualitative approach. Results and conclusions: It is extremely important to be aware of this syndrome, since the administration of injectable drugs is frequent in health services. Its early identification allows the implementation of effective treatment.
