Abstract: Ketamine and esketamine have shown promise in treating treatment-resistant depression (TRD). A total of 1,648 studies were analyzed, with 1,600 from PubMed, 8 from LILACS, and 0 from SciELO. After inclusion criteria, 30 articles suitable for the theme were selected. Intranasal esketamine demonstrated rapid and sustained efficacy in relieving depressive symptoms in TRD patients, even in non-responsive cases. Moreover, both forms of ketamine have a favorable safety profile, with generally mild and transient adverse events. Mechanistically, ketamine and esketamine induce neuroplasticity in brain regions relevant to depression, such as the prefrontal cortex, providing insights into their mechanism of action. Studies suggest that ketamine can be combined with other treatments, such as cognitive training, to maximize its benefits in treating TRD. Combining esketamine with oral antidepressants has also been shown to improve quality of life, functionality, and depressive symptoms in TRD patients.
