Abstract: Autism Spectrum Disorder (ASD) is characterized by deficits in communication and social interaction, associated with restricted and repetitive patterns of behavior. In recent years, cannabidiol (CBD) has been investigated as a therapeutic alternative for behavioral symptoms that significantly affect the quality of life of children with ASD. The present systematic review analyzed evidence from clinical trials, observational studies, case reports, reviews, and preclinical research on the use of CBD in children and adolescents with ASD. The findings demonstrate consistent improvement in irritability, aggression, and disruptive behavior, with robust results in randomized trials and observational studies. Additional benefits were observed in sleep, anxiety, and social engagement. However, the effects of CBD on the core symptoms of ASD remain uncertain. The studies suggest a favorable short-term profile, although gaps persist regarding long-term effects on neurodevelopment. Preclinical studies reinforce the biological potential of CBD, highlighting its actions on neuroinflammation, oxidative stress, and modulation of the endocannabinoid system. Thus, CBD represents a promising alternative for the management of moderate to severe behavioral symptoms in ASD, especially in cases refractory to conventional therapies.
