EFEITOS DA TERAPIA COM LASER DE BAIXA POTÊNCIA NO TRATAMENTO DE MUCOSITE ORAL EM PACIENTES SUBMETIDOS À ONCOTERAPIA DE CABEÇA E PESCOÇO
A mucosite oral (MO) consiste em uma complicação aguda proveniente da oncoterapia em pacientes com neoplasias de cabeça e pescoço. Nessa perspectiva, o uso da terapia com laser de baixa potência (LLLT), nesses indivíduos, possui capacidade de modular uma diversidade de processos metabólicos, por meio de reações fotofísicas e bioquímicas, especificando comprimento de onda, potência, tempo e energia do laser. O objetivo do presente estudo foi verificar a efetividade clínica da terapia com laser de baixa potência no tratamento de MO em pacientes oncoterápicos de cabeça e pescoço. Realizou-se uma busca nas plataformas bibliográficas Scielo, PubMed, LILACS, MEDLINE entre os anos de 2011 a 2021. Foram encontrados 21 artigos e apenas 10 atenderam a necessidade da pesquisa. Afirma-se que a MO está presente em 90-97% dos pacientes com câncer de cabeça e pescoço submetidos à oncoterapia, responsável por grandes efeitos lesivos na mucosa oral, e juntamente com o efeito de citocinas pró-inflamatórias como fator de necrose tumoral alfa (TNF-α) e interleucinas 1β, 11 e 6, que potencializam o dano tecidual. A LLLT atua nos fotorreceptores da mitocôndria, alterando o metabolismo celular e produzindo um efeito de biomodulação. Estudos em modelos animais de mucosite oral demonstraram a promoção da cicatrização de úlceras, bem como um efeito anti-inflamatório e analgésico evidenciado por uma diminuição no infiltrado de neutrófilos e, inclusive, na expressão da ciclooxigenase-2. Logo, constatou-se o aumento das evidências a favor da terapia com laser de baixa potência referentes a redução dos níveis de inflamação e dor, contribuindo para a redução da sintomatologia causada pela mucosite oral.

Abstract:
Oral mucositis (OM) is an acute complication resulting from oncotherapy in patients with head and neck neoplasms. In this perspective, the use of low power laser therapy (LLLT) in these individuals has the ability to modulate a variety of metabolic processes through photophysical and biochemical reactions, specifying the laser's wavelength, power, time and energy. The aim of the present study was to verify the clinical effectiveness of low-level laser therapy in the treatment of OM in head and neck oncotherapy patients. A search was carried out in the bibliographic platforms Scielo, PubMed, LILACS, MEDLINE between the years 2011 to 2021. Twenty-one articles were found and only 10 met the research need. It is stated that OM is present in 90-97% of patients with head and neck cancer undergoing oncotherapy, responsible for major harmful effects on the oral mucosa, and together with the effect of pro-inflammatory cytokines such as tumor necrosis factor alpha (TNF-α) and interleukins 1β, 11 and 6, which potentiate tissue damage. LLLT acts on mitochondrial photoreceptors, altering cell metabolism and producing a biomodulating effect. Studies in animal models of oral mucositis have demonstrated the promotion of ulcer healing, as well as an anti-inflammatory and analgesic effect evidenced by a decrease in the neutrophil infiltrate and even in the expression of cyclooxygenase-2. Therefore, there was an increase in evidence in favor of low-level laser therapy regarding the reduction of levels of inflammation and pain, contributing to the reduction of symptoms caused by oral mucositis.
