EFEITOS DA MASTECTOMIA NA AUTOESTIMA DE MULHERES ATENDIDAS EM UM HOSPITAL DE ALTA COMPLEXIDADE
A mastectomia é uma das intervenções mais utilizadas no tratamento do câncer de mama. É um procedimento cirúrgico de retirada total ou parcial da mama com ou sem extração de linfonodos axilares. Muitas mulheres, após a intervenção cirúrgica, apresentam problemas com imagem corporal, afetando diretamente autoestima. O estudo teve o objetivo descrever os efeitos da mastectomia na autoestima das mulheres em um hospital de Alta Complexidade. Trata-se de um estudo qualitativo com abordagem descritiva. A pesquisa ocorreu em um hospital de alta complexidade no município de Teresina-PI. Participaram da pesquisa 10 mulheres. Foram incluídas, mulheres com idade acima de 18 anos e que realizavam consultas periódicas ou estavam internadas para a retirada do cateter venoso central e, excluídas, aquelas em tratamento químio ou radioterápico. Para coleta dos dados, utilizou-se o um questionário semiestruturado. Os resultados foram organizados em categorias analíticas e analisados por meio da análise do discurso do sujeito. Evidenciou-se que a retirada da mama interferiu diretamente na autoestima das mulheres. A maioria delas relatou baixa autoestima após o procedimento. E os principais sentimentos identificados foram medo, insegurança, angústia, tristeza e dor. Espera-se com estudo contribuir com o reconhecimento dos sentimentos vivenciados pelas mulheres após a mastectomia para que medidas sejam tomadas precocemente para evitar grandes sofrimentos. É extremamente importante que os profissionais da saúde, em especial os enfermeiros, que fazem a assistência direta, reconheçam esses sentimentos e desenvolvam estratégias juntamente com a equipe multiprofissional para promover a melhoria da autoestima das mulheres e, incentivá-las a acreditar no sucesso do tratamento e no reestabelecimento da qualidade de vida.

Abstract:
Mastectomy is one of the most used interventions in the treatment of breast cancer. It is a surgical procedure of total or partial removal of the breast with or without extraction of axillary lymph nodes. Many women, after the surgical intervention, present problems with body image, directly affecting self-esteem. The study had the objective of describing the effects of mastectomy on the self-esteem of women in a High Complexity Hospital. This is a qualitative study with a descriptive approach. The research occurred in a hospital of high complexity in the city of Teresina-PI. Ten women participated in the research. We included women over 18 years of age who had periodic consultations or were hospitalized for withdrawal of central venous catheters, and excluded those undergoing chemotherapy or radiotherapy. For data collection, a semi-structured questionnaire was used. The results were organized into analytical categories and analyzed by means of subject discourse analysis. It was evident that the removal of the breast directly interfered with the women's self-esteem. Most of them reported low self-esteem after the procedure. And the main feelings identified were fear, insecurity, anguish, sadness and pain. This study is expected to contribute to the recognition of the feelings experienced by women after the mastectomy, so that measures can be taken early on to avoid great suffering. It is extremely important that health professionals, especially nurses, who provide direct assistance, recognize these feelings and develop strategies together with the multiprofessional team to promote the improvement of women's self-esteem and encourage them to believe in the success of the treatment and in the reestablishment of their quality of life.
