DILEMAS DA UNIDADE DE TERAPIA INTENSIVA: LIMITAÇÕES DO CUIDADO DE PACIENTES EM FASE TERMINAL
Este estudo estabeleceu como objetivo compreender como a equipe de saúde enfrenta os dilemas éticos das limitações de pacientes terminais. O método escolhido foi a pesquisa bibliográfica de cunho qualitativo. A busca eletrônica foi realizada considerando o período de 2018 a 2022, utilizando palavras-chave, como: "Enfermagem", "Pacientes Terminais", "Bioética", "Limitações de tratamento" e “Cuidados Intensivos”, selecionando publicações em inglês, espanhol e português, sendo que 19 artigos auxiliaram na fundamentação teórica desta pesquisa. O estudo deixou claro que incorporar os cuidados centrados na família, em conjunto com a comunicação efetiva de qualquer decisão sobre o estado do paciente, constitui um dos critérios para a qualidade do cuidado no final da vida. Convém que os profissionais de enfermagem sejam devidamente qualificados quanto às estratégias para o enfrentamento dos dilemas vivenciados na UTI. A equipe de cuidados deve estabelecer uma relação de colaboração contínua com a família, onde a comunicação eficaz, a honestidade e o respeito mútuo sejam ferramentas fundamentais ao longo de todo o processo. A atenção individualizada da família com a incorporação do valor da cultura e da espiritualidade nessa etapa é um aspecto essencial. Enfim, analisar a prática clínica de rotina em relação a um quadro de referência é o primeiro passo para melhorar os cuidados prestados no fim da vida.

Abstract:
The reason for this study was to understand how the health team faces the ethical dilemmas of the limitations of terminal patients. The method chosen was the bibliographic research of a qualitative nature. The electronic search was performed considering the period from 2018 to 2022, using keywords such as: "Nursing", "Terminal Patients", "Bioethics", "Treatment Limitations" and "Intensive Care", selecting publications in English, Spanish and Portuguese, with 19 articles helping in the theoretical foundation of this research. The study made it clear that the incorporation of family-centered care, together with the effective communication of any decision about the patient's condition, constitutes one of the criteria of quality of care at the end of life. Nursing professionals must be properly trained in coping strategies for the dilemmas experienced in the ICU. The care team must establish a continuous collaborative relationship with the family, where effective communication, honesty and mutual respect are fundamental tools throughout the entire process. The individualized attention of the family with the incorporation of the value of culture and spirituality at this stage is a fundamental aspect. Finally, analyzing the usual clinical practice against a frame of reference is the first step to improve care at the end of life.
