DIAMOND BURR ASSOCIADO A LASER VERMELHO DE BAIXA POTÊNCIA NO TRATAMENTO DE ÚLCERA DE CÓRNEA EM ÉGUA COM UVEÍTE RECORRENTE – RELATO DE CASO
Relata-se o caso de uma égua árabe de 17 anos foi examinada no Hospital Veterinário da FMVA/UNESP, diagnosticada com leptospirose em dois anos atrás, e apresenta episódios recorrentes de uveíte recorrente. Em um quadro recidivante grave bilateral neste animal, com quemose e hiperemia conjuntival intensas, além de epífora, fotofobia, blefaroespasmo e úlcera de córnea superficial na porção axial da córnea, com bordos regulares, no olho direito. Instituiu-se capa ocular protetora, tratamento sistêmico com flunixim meglumine (1,1 mg/Kg/IV/q24h, 3 dias), topicamente (q8h) Epitezan® pomada, e soro autólogo com EDTA. Após uma semana sem melhora, constatou-se que ventralmente havia um tecido irregular, caracterizando uma úlcera de bordos soltos. Optou-se, com o animal sedado com detomidina (10 µg/Kg/IV) e Anestalcon® tópico, pela realização do desbridamento com “Diamond Burr”, seguido da aplicação do laser vermelho de baixa potência (1J em 1 ponto, 35 J/cm2, uma única aplicação). Colírios (2 gotas) a base de morfina (0,1 mg/ml/q12h, 2 dias), Still® e tobramicina (q8h, 15 dias) foram acrescentados ao tratamento. Houve rápida melhora clínica, e decorridos 15 dias, havia haze corneal central discreto, que foi tratado com Maxitrol® e Lacrifilm® tópicos durante 7 dias. Enfatiza-se a rápida resolução da úlcera com esta associação de tratamentos quando comparada às outras opções terapêuticas.

Abstract:
We report the case of a 17-year-old Arabian mare who was examined at the Veterinary Hospital of FMVA/UNESP, diagnosed with leptospirosis two years ago, and has recurrent episodes of recurrent uveitis. In a severe bilateral recurrent condition in this animal, with intense chemosis and conjunctival hyperemia, in addition to epiphora, photophobia, blepharospasm and superficial corneal ulcer in the axial portion of the cornea, with regular borders, in the right eye. Protective eye cover, systemic treatment with flunixim meglumine (1.1 mg/Kg/IV/q24h, 3 days), topically (q8h) Epitezan® pomade, and autologous serum with EDTA were instituted. After a week without improvement, it was found that ventrally there was an irregular tissue, characterizing an ulcer with loose edges. We chose, with the animal sedated with detomidine (10 µg/Kg/IV) and topical Anestalcon®, to perform the debridement with “Diamond Burr”, followed by the application of the low power red laser (1J in 1 point, 35 J/cm2, a single application). Eye drops (2 drops) based on morphine (0.1 mg/ml/q12h, 2 days), Still® and tobramycin (q8h, 15 days) were provided for treatment. There was rapid clinical improvement, and after 15 days, there was discrete central corneal haze, which was treated with topical Maxitrol® and Lacrifilm® for 7 days. It emphasizes a quick resolution of the ulcer with this combination of treatments when compared to other therapeutic options.
