DIAGNÓSTICO TARDIO DE TORÇÃO TESTICULAR INTERMITENTE: UM RELATO DE CASO
INTRODUÇÃO: A dor testicular aguda é uma manifestação frequente entre crianças e adolescentes na sala de emergência. Dentre suas etiologias, a torção testicular é a condição mais grave. Esse estudo objetiva relatar um caso de torção testicular intermitente em adolescente admitido em departamento de emergência pediátrica no Brasil. METODOLOGIA: Trata-se de um relato de caso, que busca descrever a apresentação de um adolescente admitido na emergência com queixa de dor testicular aguda. RELATO DE CASO: Paciente do sexo masculino, de 15 anos, foi admitido no departamento de emergência de um centro de referência pediátrico com queixa de dor testicular à direita. Apresentava episódios prévios de dor testicular à direita há 1 ano e 6 meses, corroborando para o diagnóstico de torção testicular intermitente. Foi realizada detorção cirúrgica imediata e orquidopexia bilateral, com viabilidade testicular após quatro dias de sintomas. DISCUSSÃO: Como ocorreu com nosso paciente, episódios de dor testicular recorrentes com longos períodos de latência podem indicar o diagnóstico de torção testicular intermitente. O tratamento da torção testicular aguda é cirúrgico e envolve a avaliação de viabilidade do testículo afetado, além da realização da detorção cirúrgica imediata e orquidopexia bilateral. CONCLUSÃO: Ponderar o risco de torção testicular a partir dos dados clínicos, realizar exame físico adequado do trato geniturinário e considerar o uso de exames de imagem para estabelecer o diagnóstico diferencial são ações essenciais para redução da probabilidade de danos irreversíveis ao paciente.

Abstract:
INTRODUCTION: Acute testicular pain is a frequent manifestation among children and adolescents in the emergency department (ED). Testicular torsion is the most serious etiology. This study aims to report a case of intermittent testicular torsion in a teenager admitted to the pediatric emergency department in Brazil. METHODOLOGY: This is a reported case that seeks to describe a presentation of an adolescent complaining of acute testicular pain. CASE REPORT: A 15-year-old male patient was admitted to the ED of a pediatric referral center complaining of right testicular pain. He described recurrent right testicular pain for a 1 year and 6 months, supporting the diagnosis of intermittent testicular torsion. Immediate surgical detection and bilateral orchidopexy were performed, with testicular viability after four days of symptoms. DISCUSSION: Detection of recurrent testicular pain with long latency periods may indicate the diagnosis of intermittent testicular torsion. The treatment of acute testicular torsion is surgical and involves a feasibility assessment of the affected testis, as well as immediate surgical detection and bilateral orchidopexy. CONCLUSION: Assessing the risk of testicular torsion based on clinical history, performing adequate physical examination of the genitourinary tract and considering the use of imaging to establish differential diagnosis are essential actions to reduce the likelihood of irreversible damage to the patient.
