• Abstract: The Family Health Strategy (ESF) has become the main model for organizing Primary Health Care (PHC) in Brazil. Monitoring the evolution of the number of teams and population coverage is essential to understand PHC performance and identify structural weaknesses that could compromise its effectiveness. In this context, this study aimed to analyze the evolution of the number of Family Health teams and the estimated coverage from 2020 to 2025. For this purpose, secondary data were collected from the National Registry of Health Facilities (CNES) and the Ministry of Health's Annual Management Reports (RAG). The collected data were processed in Microsoft Excel, organized into tables showing the total number of teams per year, the coverage percentage, and the annual percentage variation, in order to identify trends of expansion, contraction, or stabilization over the historical series. The results showed significant fluctuations during the analyzed period. Between 2020 and 2021, moderate growth was observed in the number of teams and in population coverage. However, in 2022, a decline was noted, representing the largest contraction in the series. From 2023 onwards, a gradual recovery occurred, with a continuous increase in the number of teams and a return to high coverage levels, exceeding 90% in 2024 and 2025. These findings indicate the primary healthcare system's ability to reorganize after periods of instability, although they also reveal the model's vulnerability to administrative, financial, and structural challenges.
