DETERMINAÇÃO DA SUSCEPTIBILIDADE À VANCOMICINA EM ISOLADOS CLÍNICOS HOSPITALARES DE Staphylococcus aureus
Para o tratamento das infecções causadas por Staphylococcus aureus resistentes a meticilina (Methicillin-resistant Staphylococcus aureus – MRSA), os glicopeptídeos são antimicrobianos de primeira escolha, no entanto, a pressão seletiva que esses fármacos exercem, pode induzir o surgimento de isolados com sensibilidade diminuída à vancomicina, conhecidos como hVISA e VISA. O presente estudo teve por finalidade investigar a sensibilidade à vancomicina de isolados clínicos de S. aureus, no período de 2010 a 2015, de um hospital terciário do sul do Brasil. A concentração inibitória mínima (CIM) foi determinada por Etest® para 335 isolados e a análise do Perfil Populacional-Área Sob a Curva (PAP-AUC) para 80. A CIM variou de 0,5 a 8,0 µg/mL sendo que as concentrações mais frequentes foram 1,5 µg/mL com 31,9% (107/335) e 2,0 µg/mL com 29,6% (99/335). O percentual de isolados não sensíveis foi de 20,8% em 2010 a 2012 e de 17,1% em 2013 a 2015. Verificou-se que para 31,6% (12/38) das amostras obtidas entre 2010 e 2014 e que para 42,9% (18/42) das amostras de 2015, a relação PAP/AUC foi >0,9, classificando-as como não sensíveis. Verifica-se que o percentual de isolados não sensíveis à vancomicina foi elevado neste estudo, entretanto, os valores obtidos foram divergentes entre os métodos analisados. A técnica de PAP-AUC, considerada o padrão ouro, evidenciou as maiores frequências de hVISA e VISA, no entanto, é muito trabalhosa o que inviabiliza sua utilização em laboratórios clínicos de rotina. Por outro lado, embora fácil de realizar, o método de Etest® também tem suas limitações, o custo é elevado e não é tão sensível quanto o PAP-AUC para identificação de S. aureus não sensíveis, especialmente hVISA.

Abstract:
To treat Methicillin-resistant Staphylococcus aureus (MRSA) infections, glycopeptides are the antimicrobials of first choice. However, the selective pressure exerted by these drugs can lead to the emergence of isolates with reduced susceptibility to vancomycin (hVISA and VISA). This study aimed at investigating the antimicrobial susceptibility profile of S. aureus clinical isolates, from 2010 to 2015, in a tertiary hospital in southern Brazil. The minimum inhibitory concentration (MIC) was determined using Etest® for 335 isolates, and the population analysis profile-area under the curve (PAP-AUC) method, for 80 isolates. MIC values ranged from 0.5 to 8.0 µg/mL; 1.5 and 2.0 µg/mL were the most frequent concentrations, found in 31.9% (107/335) and 29.6% (99/335) of the isolates, respectively. The percentage of vancomycin non-susceptible isolates was 20.8% (from 2010 to 2012) and 17.1% (from 2013 to 2015). It was observed that for 31.6% (12/38) of the isolates obtained between 2010 and 2014, and 42.9% (18/42) of the isolates from 2015, the PAP-AUC ratio was >0.9, which allowed us to classify them as non-susceptible to vancomycin. In this study, a high rate of vancomycin non-susceptible isolates was observed, but the values differed according to the method applied. PAP-AUC, considered the golden standard, was more efficient in detecting hVISA and VISA. However, it is a very laborious test, which makes its usage impractical in routine clinical laboratories. Finally, although easy to be carried out, Etest® has limitations regarding its cost, and it is not as sensitive as PAP-AUC for the identification of vancomycin non-susceptible isolates, especially hVISA.
