Abstract: Introduction: Population aging in Brazil presents new challenges to the health system, demanding approaches that integrate the clinical, functional, and social dimensions of older adults. The Multidimensional Assessment of the Elderly (Avaliação Multidimensional da Pessoa Idosa – AMDPI), especially the Clinical-Functional Vulnerability Index (Índice de Vulnerabilidade Clínico-Funcional – IVCF-20), stands out as a strategic tool for enhancing care in Primary Health Care (PHC). Objective: To analyze, through a narrative review, the challenges and prospects of implementing the IVCF-20 in the context of the Rede Bem Cuidar (Well-Caring Network) in the state of Rio Grande do Sul, considering its impact on care management and comprehensive attention to the elderly population. Method: This is a qualitative study based on a narrative literature review, including articles, institutional documents, and official publications from databases such as SciELO, LILACS, and PubMed, as well as sources from the Ministry of Health and the Government of the State of Rio Grande do Sul. The analysis was organized into thematic axes covering the historical context of public policies, the concept and applications of the AMDPI, and the main challenges in its implementation. Results and Discussion: The IVCF-20 has proven to be a valid, accessible, and useful tool for both clinical practice and health management, especially when integrated into the Singular Therapeutic Project (PTS) and aligned with the guidelines of the Rede Bem Cuidar. However, its full implementation still faces barriers such as lack of professional training, resource shortages, limited intersectoral coordination, and institutional resistance. Conclusion: The adoption of the IVCF-20 in PHC strengthens comprehensive care for older adults and contributes to service rationalization, improved quality of life, and the sustainability of Brazil’s Unified Health System (SUS). Its consolidation depends on recognizing functional assessment as a permanent practice and promoting integration among policies, professionals, and managers.
