Abstract: Introduction: Population aging poses significant challenges and opportunities for health in the XXI century. As life expectancy increases, there is a need for innovative and sustainable approaches to promote the well-being of the elderly. This phenomenon brings challenges such as the rise in chronic diseases, long-term care demand, and social issues. Objective: To analyze and synthesize available evidence on challenges and opportunities related to population aging, with an emphasis on the health sphere, in the context of the 21st century. Methodology: This is an integrative review characterized by its descriptive nature. The analysis of collected data will be conducted qualitatively, utilizing the databases SciELO and LILACS. Results: It was observed that the inclusion of prevention programs, investments in early disease detection methodologies, monitoring of chronic conditions, implementation of personalized medical care systems, among other measures, needs to advance further to reach this population segment, instead of adopting the spontaneous demand model that places the hospital as the central piece of the system. Conclusion: The presented data emphasize the need to rethink and reformulate political and care strategies. Facing the prospect of having a more aged population than many European countries by 2050, Brazil urgently needs to create innovative and sustainable solutions. The demographic transition, though inevitable, should not be seen merely as an increase in the demand for health services but as an opportunity to promote active and healthy aging.
