Abstract: The study aims to analyze the epidemiological profile and the incidence rate of HIV/AIDS in the elderly population in Brazil, from 2011 to 2021. It is an epidemiological, retrospective and quantitative study, using data from the Department of Informatics of the Unified Health System (DATASUS) provided by the Information System for Notifiable Diseases (SINAN). The results indicate an increase in the detection rate of the disease among the elderly population, with the majority having up to seven years of study. This increase contrasts with the decrease in diagnoses in the younger population. Regionally, the Southeast region had the highest number of cases, while the Central-West had the fewest. The 'heterosexual' category was the main route of exposure. It is concluded that, despite population aging and advances in the medical field, sexuality in the elderly population is insufficiently addressed in public policies, among health professionals and by the elderly themselves, making them more susceptible to infections by sexually transmitted diseases. The importance of addressing this issue goes beyond promoting the health and well-being of the elderly population, and it is crucial to understand the factors that contribute to this reality, which presents itself as an emerging challenge for public health.
