Abstract: The essay discusses and analyzes the two clinical approaches used in primary health care in the Unified Health System, the traditional Clinic and the expanded Clinic, using as inspiration the article entitled "Clinic: the art of balancing the disease and the subject", by Rubens Bedrikow and Gastão Wagner de Sousa Campos (2011). Through a critical look at social conditions, based on the historical, political and economic context, the potential and weaknesses of the two models of clinical practice were investigated. Through this analysis, the need for a perspective that overrides the simplistic positivist view in the field of medicine was exposed, recognizing the importance of considering broader aspects of the patient. Despite the merits of the traditional approach in high-complexity intensive care centers of the Unified Health System, it is not sufficient to meet the social health needs of users. Faced with this limitation, the National Humanization Policy, instituted in 2004, presents the extended Clinic as an approach that does not disregard the experience of illness and the historical context in which this experience is situated. The essay also reflects on the close link between macroeconomic policies and models of health care provision in Primary Care. As these issues are explored, a challenge emerges: how can we balance tradition and innovation, authority and autonomy, justice and diversity in the search for medical care in Primary Care that meets the social health needs of users while taking care not to medicalize any and all social issues that arise?
