Abstract: This chapter addresses population aging from a functional perspective, highlighting the need to transform healthcare practices in response to the growing demands of the elderly population. Initially, it discusses the transition from a biomedical model centered on disease to a broader approach oriented towards maintaining autonomy and social participation. Fundamental concepts are presented, such as the distinction between senescence and senility, as well as the importance of a multidimensional assessment of the elderly, encompassing physical, cognitive, emotional, and social aspects. In this context, the International Classification of Functioning, Disability and Health (ICF) is introduced as an essential theoretical model for an integrated understanding of functionality. Throughout the chapter, current evidence in physiotherapy in gerontology is discussed, with emphasis on the practical application of the ICF in clinical reasoning and the development of individualized interventions. The main components of clinical-functional assessment are explored, including motor, cognitive, and emotional aspects, as well as the factors that influence adherence to physiotherapy treatment. Furthermore, the role of the social support network in maintaining functionality and promoting healthy aging is highlighted. The purpose of this chapter is to offer an integrated and person-centered perspective on the elderly, contributing to the improvement of physiotherapy practice and the promotion of more effective, humanized, and evidence-based care.
