CULTURA DE ORIENTAÇÃO AO ERRO: EXPLORANDO PERCEPÇÕES NA ATENÇÃO BÁSICA DA SAÚDE
A preocupação com os erros humanos no campo da saúde não é recente e não apresenta uma solução fácil para uma problemática complexa. As políticas de Segurança do Paciente e Qualidade em Saúde vêm produzindo orientações e estratégias para enfrentar um problema que é inerente à condição humana. “Errar é humano”. Esta máxima nos coloca em uma dialética de êxito e medo. A atenção com a qualidade e segurança do paciente está em todas as ações de saúde, podendo ser considerada como elemento determinante e estruturante das Políticas de Atenção Básica. Neste âmbito, incentivar a criação de uma cultura de aprendizagem baseada em práticas, possibilita a promoção de uma cultura justa que incentive a inovação e reduza os prejuízos causados por comportamentos inadequados. Este trabalho tem como objetivo analisar a percepção sobre a cultura de orientação do erro, em profissionais que atuam na atenção básica da saúde no município do Rio de Janeiro. Observou-se neste estudo, que apesar das políticas de Qualidade e Segurança do Paciente já serem bastante difundidas e reconhecidas nos ambientes de saúde e conjugadas com várias pesquisas existentes sobre o tema, as respostas encontradas reforçaram a ideia da tendência a “ocultação” dos erros para evitar sanções ou punições. Faz-se necessária uma mudança neste paradigma, de forma que o capital social possa favorecer a aprendizagem evitando a omissão dos eventos adversos nos ambientes com valores de medo e punição.

Abstract:
The Concern about human errors in health is not recent and does not provide an easy solution to a complex problem. Patient Safety and Health Quality policies have been producing guidelines and strategies to address a problem that is inherent to the human condition. "To err is human." This maxim puts us in a dialectic of success and fear. Attention to patient quality and safety is in all health actions, and can be considered as a determining and structuring element of Primary Care Policies. In this context, encouraging the creation of a practice-based learning culture enables the promotion of a fair culture that encourages innovation and reduces the harm caused by inappropriate behavior. This paper aims to analyze the perception about the error orientation culture, especially regarding the aspect of sharing the error at work, in professionals who work in primary health care in the city of Rio de Janeiro. In this study, it was observed that despite the policies of Quality and Patient Safety already widespread and recognized in health environments, combined with several researches performed in the area the responses reinforced the idea of the tendency to “hide” errors to avoid sanctions or punishments. A change in this paradigm is necessary, so that social capital can favor learning by avoiding the omission of adverse events in environments with values of fear and punishment.
