CUIDADOS PALIATIVOS NAS UNIDADES DE TERAPIA INTENSIVA: UMA REVISÃO NARRATIVA DA LITERATURA
Introdução: a Unidade de Terapia Intensiva (UTI) oferece suporte de vida avançado a pacientes instáveis com potencial de recuperação. O problema da medicina intensiva consiste em oferecer recursos tecnológicos de custo elevado para manutenção artificial da vida em vigência da doença avançada, em progressão e com pouca chance de reversibilidade, prolongando o processo de morrer de forma lenta, sofrida e sem qualidade de vida, com elevados custos psicológicos, sociais e financeiros para os envolvidos (paciente, família, equipe e hospital). Neste contexto, o presente trabalho foi desenvolvido com o objetivo de realizar uma revisão narrativa da literatura sobre Cuidados Paliativos em UTI. Revisão da literatura: Cuidado paliativo é uma abordagem multiprofissional que visa a melhoria da qualidade de vida de pacientes e familiares que enfrentam doenças ameaçadoras da vida, através da prevenção e do tratamento precoce dos sintomas de sofrimento físico, psíquico, espiritual e social, que devem ser prestados em concomitância aos cuidados curativos, com intensidade individualizada conforme a vontade dos pacientes e famílias e com a evolução própria de cada doença. A realidade do tratamento nas UTIs reforça a necessidade da implantação de ações integradas de cuidados paliativos nessas unidades. Conclusão: a implantação de ações integradas em Cuidados Paliativos na UTI traz a possibilidade de exercer a ética em sua plenitude, de morrer sem sofrimento, sem medidas extraordinárias inúteis ou fúteis, com respeito ao ser humano e seus familiares no fim de vida, impedindo o retardamento da morte inevitável com redução dos elevados custos psicológicos, sociais e financeiros produzidos pela obstinação terapêutica.

Abstract:
Introduction: The Intensive Care Unit (ICU) offers advanced life support to unstable patients with recovery potential. The problem of intensive care medicine is to offer high-cost technological resources for the artificial maintenance of life in the presence of an advanced disease, in progression and with little chance of reversibility, prolonging the process of cruelly dying, with high psychological, social and financial costs for those involved (patient, family, staff and hospital). In this context, the present work was developed with the objective of carrying out a narrative review of the literature on Palliative Care in the ICU. Literature review: Palliative care is a multidisciplinary approach that aims to improve the quality of life of patients and families facing life-threatening diseases, through the prevention and early treatment of symptoms of physical, psychological, spiritual and social suffering, which should be provided concomitantly with paliative care, individualized intensity according to the wishes of patients and families and with the evolution of each disease. The reality of treatment in the ICUs reinforces the need to implement integrated palliative care actions in these units. Conclusion: the implementation of integrated actions in Palliative Care in the ICU brings the possibility for physicians to exercise ethics in its fullness, as well as the possibility for pacient to have a painless death without the use of pointless or futile measures, It is vital to respect the person and their family members at the end of their livre by stopping the delaying of na inevitable death. Additionally, reducingin the high psychological, social and financial costs produced by therapeutic obstinacy.
