CUIDADOS DE ENFERMAGEM AO PACIENTE EM VENTILAÇÃO MECÂNICA INVASIVA: REVISÃO INTEGRATIVA
Objetivou-se descrever os cuidados de enfermagem durante a assistência ao paciente em ventilação mecânica invasiva. Consiste em uma revisão integrativa da literatura, de caráter descritivo com abordagem qualitativa, realizada na Biblioteca Virtual de Saúde (BVS) nas bases de dados MEDLINE, BDENF e LILACS em março e abril de 2019. Os Descritores em Ciências da Saúde (DeCS) foram: Cuidados de Enfermagem; Assistência ao Paciente; Respiração Artificial. Foi realizado o cruzamento entre os descritores utilizando o operador booleano AND e aplicado os critérios de inclusão: Texto completo disponível; tipo de documento artigo; idiomas inglês, português e espanhol; anos de publicação de 2011 à 2018, período correspondente aos últimos oito anos disponíveis na biblioteca, que resultaram em 40 artigos. Após a leitura completa sete artigos atenderam ao escopo dessa pesquisa. Foi observado uma atenção quanto à manutenção do ventilador mecânico, remoção de sujidades e troca de filtro/umidificador e do circuito; cuidados voltados a higiene oral do paciente, hidratação, avaliação da língua, saliva, mucosa, gengiva e dentes, posicionamento da cabeceira do leito e mudança de decúbito. Observou-se também, a verificação frequente do cuff traqueal, a troca e/ou fixação do cadarço todos os dias; a utilização de gazes nas laterais da cavidade oral quando se percebia alguma agressão à pele do paciente; o monitoramento frequente dos sinais vitais e a aspiração subglótica com técnica asséptica, com o intuito de prevenir complicações. Apesar do cuidado ao paciente intubado acontecer da maneira como preconiza a literatura pertinente, ainda se nota lacunas acerca de estudos que analisem a acurácia desses procedimentos para a prevenção de IRAS em pacientes criticamente enfermos e que necessitam de suporte ventilatório invasivo. Ademais, verificou-se a importância de padronizar a assistência oferecida a esses pacientes para realização de um cuidado eficaz.

Abstract:
The objective of this study was to describe nursing care during assistance to patients on invasive mechanical ventilation. It consists of an integrative literature review, of a descriptive character with a qualitative approach, carried out in the Virtual Health Library (VHL) in the MEDLINE, BDENF and LILACS databases in March and April 2019. The Health Sciences Descriptors (DeCS) were : Nursing care; Patient Assistance; Artificial Respiration. The descriptors were crossed using the Boolean operator AND and the inclusion criteria were applied: full text available; type of document document; English, Portuguese and Spanish languages; years of publication from 2011 to 2018, corresponding to the last eight years available in the library, resulting in 40 articles. After the complete reading, seven articles met the scope of this research. Attention was paid to maintaining the mechanical fan, removing dirt and changing the filter / humidifier and the circuit; care focused on the patient's oral hygiene, hydration, evaluation of the tongue, saliva, mucosa, gums and teeth, positioning the head of the bed and changing the position. It was also observed, the frequent verification of the tracheal cuff, the exchange and / or fixation of the lace every day; the use of gauze on the sides of the oral cavity when any aggression to the patient's skin was perceived; frequent monitoring of vital signs and subglottic aspiration with aseptic technique, in order to prevent complications. Although care for intubated patients occurs as recommended by the relevant literature, there are still gaps regarding studies that analyze the accuracy of these procedures for the prevention of HAIs in critically ill patients who require invasive ventilatory support. In addition, it was verified the importance of standardizing the assistance offered to these patients in order to provide effective care.
