CUIDADO PÓS-TRANSFUSIONAL NA UNIDADE DE TERAPIA INTENSIVA NEONATAL: UM RELATO DE EXPERIÊNCIA
A hemoterapia atualmente se constitui em uma das alternativas terapêuticas mais efetivas no tratamento de determinadas patologias e na reposição de hemocomponentes e hemoderivados essenciais à manutenção da vida. Quando se trata particularmente da hemotransfusão realizada na Unidade de Terapia Intensiva Neonatal (UTIN), o cuidado de enfermagem é essencial na segurança desse paciente, pois, a hemotransfusão em Neonatologia tem uma abordagem diferencial daquela do adulto, devido às seguintes características: maior sensibilidade ao frio; maior risco de anóxia tecidual; imaturidade metabólica e imunológica, fisiologia hematológica peculiar e patologia própria do Recém-Nascido (RN). Portanto, o enfermeiro deve ter conhecimentos, sobre a hemotransfusão e os cuidados e reações pós transfusão sanguínea. O objetivo geral é relatar a experiência de acadêmicos de enfermagem, acerca dos cuidados pós-transfusional em uma Unidade de Terapia Intensiva Neonatal. Trata-se de um estudo descritivo, de abordagem qualitativa do tipo relato de experiência foi realizado no mês de setembro de 2019 por dois acadêmicos de enfermagem do Centro Universitário INTA-UNINTA. Durante as vivências práticas da disciplina de UTI, em uma Unidade de Terapia Intensiva Neonatal de um hospital de referência na zona norte do estado do Ceará. Essa equipe juntamente com a enfermeira e alunos, conferiam e checavam todos os dados, antes de infundir o sangue no paciente. E logo ao término da infusão, os alunos juntamente com a enfermeira do setor, realizavam a aferição dos sinais vitais do RN, pois o controle desses sinais funciona como parâmetro para as verificações anteriores, podendo trazer informações significativas sobre o estado do paciente. Portanto a experiência relatada, na UTIN acerca da hemotransfusão, possibilitou uma melhor compreensão dos alunos acerca do assunto despertando assim possíveis estudos sobre a temática, além de enfatizar a importância do cuidado de enfermagem, principalmente do tocante pós-transfusional.

Abstract:
Hemotherapy is currently one of the most effective therapeutic alternatives in the treatment of certain pathologies and in the replacement of blood components and blood products essential to the maintenance of life. When it comes particularly to blood transfusions performed in the Neonatal Intensive Care Unit (UTIN), nursing care is essential in the safety of this patient, because blood transfusions in Neonatology have a different approach from that of adults, due to the following characteristics: greater sensitivity to cold; increased risk of tissue anoxia; metabolic and immunological immaturity, peculiar hematological physiology and pathology specific to the Newborn (RN). Therefore, the nurse must have knowledge about blood transfusion and care and reactions after blood transfusion. The general objective is to report the experience of nursing students about post-transfusion care in a Neonatal Intensive Care Unit. This is a descriptive study, with a qualitative approach of the type of experience report, which was carried out in September 2019 by two nursing students from Centro Universitário UNINTA. During the practical experiences of the ICU discipline, in a Neonatal Intensive Care Unit of a referral hospital in the north of the state of Ceará. This team, together with the nurse and students, checked and checked all data before infusing the patient with blood. And at the end of the infusion, the students, together with the nurse in the sector, performed the measurement of the newborn's vital signs, as the control of these signs works as a parameter for the previous checks, and can bring significant information about the patient's condition. Therefore, the experience reported in the UTIN about blood transfusion, enabled a better understanding of students on the subject, thus awakening possible studies on the theme, in addition to emphasizing the importance of nursing care, especially in the post-transfusion area.
