Abstract: Aging is associated with physiological, musculoskeletal and neurological dysfunctions, such as sarcopenia and cognitive impairment. In this sense, non-pharmacological strategies such as resistance training in combination with creatine supplementation have been studied to improve this condition, especially sarcopenia. Objectives: To elucidate the role of creatine combined with resistance training in reducing muscle mass, strength loss and age-related neuroinflammation. Methodology: This study is a narrative literature review. The search was conducted in the PubMed database for articles from 2020 to 2025, with participants aged 49 years and older who performed resistance training with and without creatine supplementation. The most recent guidelines were used. The exclusion criteria were: original articles and meta-analyses prior to 2020; age group below 49 years; books; incomplete and/or unavailable articles. Results: Creatine has a potentiating effect on neuromuscular adaptations, with reduced inflammation and increased cognitive function, especially when combined with resistance training. Discussion: these results were achieved when using a saturation phase followed by doses of 5 g/day or more, possibly caused by a greater reduction in the interstitial creatine of these elderly individuals. Conclusion: creatine supplementation appears to be a safe non-pharmacological strategy for reducing neuromuscular and cognitive impacts of aging when combined with resistance training.
