Abstract: Obstructive sleep apnea (OSA) is a multifactorial respiratory disorder related to obesity and anatomical malformations. This study analyzed 30 scientific publications on the correlation between obesity, structural alterations and OSA, investigating the metabolic and cardiovascular impacts of the disease. The results highlight that obesity is one of the main risk factors for OSA, with a significant reduction in the severity of the condition after weight loss. Anatomical malformations, such as tonsil and adenoid hypertrophy and retrognathia, have also been associated with the development of OSA, with adenotonsillectomy being an effective therapeutic option. The disease is strongly associated with cardiovascular complications, such as hypertension and adverse cardiac remodeling. Multidisciplinary treatment, including CPAP, surgery and new pharmacological therapies, has proven to be essential for the effective management of the condition. The implementation of personalized approaches is essential to optimize clinical outcomes of patients with OSA.
