Abstract: Dens Invanginatus, also known as dens in dens, is characterized as an anomaly that occurs at the time of development of dental enamel, the best known causes are genetic or environmental factors, being recommended for its diagnosis and treatment plan to request 2D and 3D radiographic exams. . With endodontic surgery it is possible to remove necrotic and infected remains that have lodged in the apical portion of the tooth. The aim of this study was to report a clinical case of endodontic surgery of a dens invaginatus, with the use of PRF graft and lyophilized bovine bone. The use of PRF graft in this surgery is justified by its biological properties, generating deposition of growth factors, performing cell migration, proliferation and differentiation, its gelatinous characteristic allows better handling and adaptation of the material in the region. The anatomic complexity of both the external and internal dens invaginatus makes its structure fragile. To obtain a better choice in the treatment plan, it is essential to evaluate the conditions that the pulp and its root development are in. For a better assessment of the healing of the periapical lesion after apicoectomy, follow-up was indicated for 1 year after surgery with the use of 3D technology, volumetric computed tomography.
