Abstract: Introduction: The use of electronic cigarettes among adolescents and young adults is considered a worldwide public health problem. Exacerbated consumption accelerates inflammatory and infectious processes in the respiratory and cardiovascular systems, contributing to the emergence of pathologies and increasing the morbidity and mortality of patients in the long term. Thus, as a preventive and treatment measure, rehabilitation is sought as an adjunct to intensive drug treatment, aimed at cessation and quality of life. Objective: To discuss the impact of electronic cigarettes versus conventional cigarettes on the manifestation of cardiorespiratory diseases. It also aims to describe the beneficial potential of physical rehabilitation in smoking cessation. Methods: Systematic literature review, using the PRISMA diagram model adapted from 2020 for indexed platforms such as PubMed, ScienceDirect, PEDro and Cochrane Library, including systematic reviews, meta-analysis, clinical trials and prospective studies, without language restrictions, published between January 2018 and February 2024. With electronic and conventional cigarette users, as well as physical rehabilitation via aerobic exercise, walking tests and isometrics for more than 2 weeks. Results: In summary, the findings show that electronic cigarettes are more harmful to health than conventional cigarettes. It is estimated that 70% of pulmonary and cardiac impairment is evident on admission of chronic patients to the intensive care unit. Cardiac rehabilitation is indeed beneficial for patients who use electronic cigarettes, if it is carried out on a regular basis. Conclusion: In summary, e-cigarettes, when compared to conventional cigarettes, are the biggest cause of cardiovascular pathologies due to their excessive consumption, indiscriminate sale and toxic compounds similar to the poisoning process. Thus, cardiac rehabilitation has emerged as an ally in the primary to tertiary sphere in the treatment and preventive process of smokers and patients with heart disease, demonstrating levels of effectiveness in smoking cessation, improvement in cardiorespiratory fitness and greater quality of life during exercise.
