Abstract: Tension-type headache (TTH) is the most prevalent form of primary headache, with high global incidence and significant impact on quality of life. Its pathophysiology involves the interaction between peripheral mechanisms, such as myofascial trigger points and the release of inflammatory mediators, and central processes, including neurochemical alterations and pain sensitization. Evidence highlights the role of inflammatory pathways and oxidative stress, with increased levels of pro-inflammatory cytokines and reactive oxygen species perpetuating the painful condition. Therapeutic strategies include the use of drugs such as analgesics, anti-inflammatory agents, and antidepressants, combined with non-pharmacological interventions such as physiotherapy, relaxation techniques, biofeedback, and lifestyle modifications. Thus, TTH should be understood within a biopsychosocial framework, with a multidisciplinary approach being essential for prevention, pain control, and improvement of patients’ quality of life.
