Abstract: Hemostasis results from the balance between pro- and anticoagulant factors, which act in the formation and degradation of clots to protect the circulatory system. However, pathological conditions can generate thrombi and emboli, obstructing vessels, compromising tissue perfusion and causing ischemia and infarction. The severity of these events depends on the extent of the obstruction and the affected area. Anticoagulant therapy is essential to prevent thromboembolic complications resulting from clinical conditions that increase the risk of their occurrence. Warfarin is the main oral anticoagulant used in Brazil. Warfarin management requires individualization of the dose, continuous monitoring through the International Normalized Ratio (INR) and management of drug and food interactions, in addition to good adherence to treatment. The present study aimed to characterize the sociodemographic, clinical and pharmacotherapeutic profiles of patients using warfarin treated at the Anticoagulation Outpatient Clinic of Hospital Risoleta Tolentino Neves (HRTN). This is an observational, descriptive study. Data were collected through structured interviews and analysis of medical records, covering sociodemographic, clinical and pharmacotherapeutic information. A total of 212 participants were characterized, with a predominance of women (58.5%), patients aged 60 years or older (58.0%), who did not complete high school (55.6%), using multiple medications (55.3%) and with comorbidities (58.5% with three or more). It was observed that a significant portion of the patients had time in therapeutic range (TTR) of less than 60%, indicating difficulties in controlling anticoagulation. The findings suggest that the identified profile may be associated with significant challenges in adherence to and management of warfarin therapy. Factors such as comorbidities, polypharmacy and gaps in health education compromise the effectiveness of treatment. Personalized interventions, which consider the sociodemographic, clinical and pharmacotherapeutic characteristics of patients, are essential to improve therapeutic outcomes. In addition, health education strategies can contribute to patient empowerment, favoring adherence to treatment and therapeutic safety.
