Abstract: Polycystic ovary syndrome (PCOS) is a common endocrine condition affecting about 5-7% of women of reproductive age, being a leading cause of female infertility. The study used an integrative literature review, analyzing articles from PubMed, SciELO, and LILACS. Twenty-four articles addressing in vitro fertilization in women with PCOS, published between 2013 and 2023, in English or Portuguese, were selected. It is characterized by hyperandrogenism, oligo/anovulation, and polycystic ovaries. PCOS significantly impacts the quality of life and reproductive health of affected women, leading to complications such as obesity, hirsutism, and cardiovascular disorders. In recent years, In Vitro Maturation (IVM) has emerged as an alternative to conventional in vitro fertilization (IVF) for women with PCOS, offering advantages such as less ovarian stimulation, shorter treatment duration, and greater patient convenience. Vitamin D supplementation has been studied as an intervention to improve insulin resistance and lipid metabolism in women with PCOS, but its role in in vitro fertilization remains uncertain. Women with PCOS face a higher risk of obstetric complications in pregnancies resulting from assisted reproductive treatments, such as IVF, including pre-eclampsia. Furthermore, PCOS is associated with irregularities in ovarian angiogenesis, which can affect oocyte maturity and fertility treatment effectiveness. Studies have evaluated different treatment strategies for women with PCOS undergoing IVF, such as the administration of clomiphene citrate and vaginal progesterone. The choice of appropriate treatment protocol is crucial to balance obtaining a sufficient number of viable oocytes with the risk of ovarian hyperstimulation syndrome (OHSS). In summary, careful and individualized management of IVF treatment in women with PCOS is essential to optimize success rates and minimize associated risks.
