Abstract: Heart failure with preserved ejection fraction (HFpEF) is a complex syndrome characterized by preserved left ventricular ejection fraction (LVEF) above 50%, with symptoms of congestive heart failure despite seemingly preserved ventricular function. This condition is commonly associated with comorbidities such as obesity, hypertension, and diabetes mellitus, complicating clinical management due to patient heterogeneity and variable response to available therapies. Recent studies, like STEP-ICCFEP, have explored the role of agents such as semaglutide in improving symptoms and exercise function, suggesting benefits through systemic inflammation reduction. Strategies such as early therapeutic intensification based on biomarkers like NT-proBNP have shown promise in managing HFpEF, underscoring the need for personalized approaches to optimize clinical outcomes. The incorporation of sodium channel blockers like flecainide and propafenone has also been investigated, offering potential in reducing arrhythmias and maintaining sinus rhythm in HFpEF patients, although safety and efficacy challenges need careful consideration for precise and safe clinical application. This summary synthesizes the primary challenges and advances in HFpEF management, emphasizing the importance of ongoing research to enhance the management of this complex cardiovascular condition
