Abstract: Primary Health Care is the place of first contact of individuals in a community with the health system. The objective of this study is to develop a coherent action plan that can efficiently contemplate the prevention of Systemic Arterial Hypertension in the area covered by the Tocoiós de Minas Family Health Strategy team in the municipality of Francisco Badaró. This is a practical intervention study supported by the situational strategic planning of the territory served by the health team. An evaluation of local problems was carried out and the main problem was identified in order to develop a causal determination network. The results showed a percentage of the hypertensive population of 20.2%. Hypertension is the most frequent modifiable risk factor for coronary heart disease identified in studies in Brazil and other countries, and its adequate treatment significantly reduces mortality and cardiovascular morbidity. Health promotion and prevention actions are of great relevance and essential in primary care and its planning focused on the control and reduction of risk factors of Systemic Arterial Hypertension seeks more satisfactory results and better quality of life of the population.
