Abstract: VL (Visceral Leishmaniasis) is the most severe form of leishmaniasis, due to frequent complications and high mortality rates if left untreated. In the state of Maranhão, 669 cases were recorded with an incidence of 9.8 cases per 100,000 inhabitants and a lethality rate of 7.5% in the year 2022. In this context, the present study aimed to analyze the epidemiological profile and spatial dynamics of LV in the state of Maranhão. Therefore, a descriptive study with a quantitative approach was conducted. Data were collected from LV notifications in the state of Maranhão, considering the number of cases recorded in the Department of Informatics of the Unified Health System - DATASUS from 2010 to 2020. Using Microsoft Office® Excel, the incidence rates of cases were calculated, and maps were constructed using QGIS version 3.10. Variables such as age group, race, gender, HIV coinfection, parasitological diagnosis, and case outcomes were analyzed. A total of 6,259 human LV cases were reported during the evaluated period, with the following profile: male (64.3%), self-declared mixed-race (77.3%), age group 1 to 4 years (32.4%), incomplete elementary education (10.6%), and urban residents (64.4%). The majority were new cases (91.4%), with HIV coinfection (9.8%), evolving to cure (60.6%) or death (7%). The highest incidence rates of LV were observed in the municipalities of Porto Franco, Timbiras, Presidente Dutra, and Imperatriz. The data presented in the study demonstrated the Maranhão municipalities with the highest incidence rates of LV, as well as the profile of individuals susceptible to this infection during the study.
