Abstract: Children with Special Health Needs are a group of children who have fragile health conditions and require services more frequently than the general population presented by the child population. Given the specificities, it is common for them to require frequent hospital admissions. The nurse is the professional most present in direct hospital care for patients, sometimes facing challenges in dehospitalizing these children. This is an integrative review carried out in the databases: PubMed, BVS, CINHAL, EMBASE and Web of Science aiming to identify evidence in the literature related to nurses' role in hospital discharge of Children with Special Health Needs. 15 articles made up the sample. of the study. Two categories emerged from the analysis: action through care practices (technical training of family members and caregivers; construction of new technologies, validation of instruments for care and carrying out home visits after hospital discharge) and action through management practices (evaluation of hospital indicators; preparation protocols, flows, routines and development of new professional skills). It was concluded that nurses have a fundamental role in assistance and management to carry out strategic actions for the hospital discharge of children with special health needs, working in different facets of care, from guidance to the development of new workflows.
