Abstract: Introduction: Pregnancy is a natural process, but some pregnant women may develop conditions that increase gestational risk, making specialized prenatal care essential. Despite advances in maternal and child health policies, maternal mortality is still a significant challenge, reinforcing the need for qualified care. Primary Health Care (PHC) plays a central role in this care, being the main gateway for pregnant women. However, difficulties related to access and referral to specialized care are still common. In order to improve this flow of care, outpatient care under the PASA, model seeks to integrate PHC with specialized care, ensuring continuity and comprehensiveness in maternal and child care. Objective: To analyze the nursing care provided to medium- and high-risk pregnant women at the Specialized Care Outpatient Clinic (AAE) in the municipality of Floriano-PI. Methodology: Case report with a qualitative and descriptive approach, carried out at the AAE of the Tereza Chaib Polyclinic. Participant observation of the care provided by the nurses, documentary analysis of medical records and institutional protocols and the Content Analysis technique were used to systematize the data. The triangulation of information ensured methodological rigor and provided a broader view of the care practices adopted at the service. Results: It was found that the specialized outpatient clinic has a multi-professional team, in which nurses play an essential role. Their duties include initial reception, risk stratification, nursing consultations and continuous monitoring of pregnant women. The Systematization of Nursing Care (SNC) is widely used to structure care, guaranteeing comprehensive and humanized care. The care flow adopted allows for the organization of consultations and the definition of individualized care plans, optimizing the quality of care. However, challenges were identified in terms of accessibility, the flow of referrals and the lack of specific regulations to strengthen the role of nurses in secondary care. Conclusion: The care provided at the Floriano-PI EAC is essential to guaranteeing qualified prenatal care, promoting integration between PHC and specialized care. Nurses play a strategic role in coordinating care, strengthening maternal and child care through the SNC and educational activities. However, in order to optimize this model of care and reduce gestational complications, it is necessary to improve the regulation of the flow of care and ensure greater regulatory support for the work of nurses in secondary care.
