ASSOCIAÇÃO DE IMUNONEFRITE RELACIONADA A IMUNOTERAPIA NO TRATAMENTO DO ADENOCARCINOMA DE PULMÃO METASTÁTICO: RELATO DE CASO
INTRODUÇÃO: O Adenocarcinoma de pulmão, responsável por cerca de 40% de todos os cânceres pulmonares, cujo tratamento na doença avançada tornou se promissor com a introdução da imunoterapia associada a quimioterapia. Todavia, casos de nefrite imunomediada são raros e pouco relatados, cerca de 1,7 % dos pacientes em estudo clínico. OBJETIVOS: Relatar um caso de adenocarcinoma de pulmão, cujo paciente apresentou imunonefrite imunomediada. CONCLUSÕES: O caso clínico chama atenção pela gravidade e precocidade da nefrite imunomediada associada a endocrinopatia, apesar da expectativa de alta taxa de resposta, paciente apresentou pseudoprogressão em seus primeiros exames de reestadiamento. A manifestação ativa foi relatada a indústria farmacêutica como evento de fármaco vigilância e segue em acompanhamento sem repercussões graves ao paciente.

Abstract:
INTRODUCTION: Lung adenocarcinoma, responsible for about 40% of all lung cancers, whose treatment in advanced disease has become promising with the introduction of immunotherapy associated with chemotherapy. However, cases of immune-mediated nephritis are rare and somewhat related, about 1.7% of patients in clinical study. OBJECTIVES: To report a case of lung adenocarcinoma, whose patient had immune- mediated immune nephritis. CONCLUSIONS: This case calls attention to the gravity and precocity of immune- mediated nephritis associated with endocrine disease, despite the expectation of high response rates, the patient had pseudoprogression in his early restaging tests. An active manifestation was related to the pharmaceutical industry as drug surveillance and is being followed up without serious repercussions for the patient.
