ASPECTOS BIOÉTICOS EM CIRURGIA BARIÁTRICA NA ADOLESCÊNCIA

Abstract: ABSTRACT: Although dietary treatment and lifestyle changes are the basis for managing childhood obesity, bariatric surgery in adolescents is becoming an important strategy in the management of young patients who are unable to control their excess weight. Objective: to identify, based on the literature, bioethical aspects related to bariatric surgery in adolescents. Method: this is an integrative literature review study that used the adapted recommendations of the Preferred Reporting Items for Systematic Reviews and Meta-Analyses from the SciELO, Lilacs and Medline databases. Results: the search strategy identified 46 texts, from which after applying the eligibility criteria – texts that necessarily mentioned the ethical or bioethical aspects of bariatric surgery in adolescents – a sample of ten texts was selected. The discussion was organized into thematic axes that covered the bioethical themes related to bariatric surgery in adolescence, namely: Axis I. beneficence and non-maleficence (risks versus benefits); Axis II. autonomy and informed consent; and Axis III: stigma and prejudice. Conclusion: it was observed that the indications and contraindications of bariatric surgery in adolescence should be known and analyzed in each case. However, for some adolescents, surgery may be the only way to treat obesity and more severe comorbidities – since in approximately 90% of patients, comorbidities improve or are resolved after surgery. Regarding bioethical aspects, the biggest problems related to bariatric surgery are in the autonomy and informed consent of the adolescent, since there is still a lack of knowledge regarding the subject associated with the patient's lack of maturity and the disharmonious relationships of family members (parents or guardians), which generate a series of challenges in assessing the interests of adolescents.
